Sliding Scale Fee Discount Available for Medical, Behavioral Health and Specified Dental Services

As part of Kodiak Area Native Association’s vision that the best quality service be available to all people on Kodiak Island, KANA now offers a sliding fee discount for patients and members of their families who fall below 200% of the poverty guidelines as set forth in the Alaska Federal Poverty Level; which is updated annually. The sliding fee discount is offered for Medical, Behavioral Health, and specified dental services. Income levels are based on total family income and size.

At registration, patients will be asked for brief income information which will be used to determine possible eligibility for the Sliding Fee Discount Program.  Patients who qualify and wish to apply for the Sliding Fee Discount Program will be required to complete a more detailed application and provide verification of income. Patients who choose to submit an application for a Sliding Fee Discount must provide all required information, including gross income and family size.

In order to verify gross family income, the patient must present at least one document for all applicable household members; a list of documents that KANA accepts to verify income can be acquired at the front desk. A patient may initially self-report gross family income at the first visit but will be required to provide supporting documentation within 30 days or before their next visit or family member’s visit, whichever occurs first, if they wish to continue to receive the discount.
