Every three years, KANA partners with Providence Kodiak Island Medical Center to administer a very important survey in the Kodiak community. The Community Health Needs Assessment solicits information regarding the health needs and concerns of all our Kodiak communities. The results help to shape the local healthcare services and programs that we and other healthcare providers offer to the community. 

In order to get an accurate reflection of the communities' needs, we will be administering the survey to as many people as possible using either a paper or electronic methodology through June 10, 2016. It is important to hear from people representing all the diverse groups of Kodiak, including the outlying areas and remote communities. As an incentive, people who complete surveys are entered into a drawing for prizes to include gift cards, and electricity and fuel vouchers.

This survey data is very important to Kodiak, especially in these times of diminishing state and federal funding. We need solid data to identify and demonstrate need, not only for direct funding where it is most useful, but also to attract grant funding for Kodiak health initiatives. We are interested in your health care needs and would like to get your opinions in order to help improve the health of the Kodiak community.

This survey is anonymous and we will not ask for your name. Please take the survey to contribute your experience to the Community Health Needs Assessment by June 10. Quyanaa!
